We manage your personal information in accordance with applicable privacy laws. This policy outlines the principles and practices we follow in protecting your personal information.
What is personal information?
Personal information means information about an identifiable individual. This includes things like your name, home address and phone number, age, identifying numbers, financial information, educational history, etc.

We collect only the personal information that we need for the purposes of providing services to our clients, including personal information needed to:

However, we don’t provide this notification when you volunteer information for an obvious purpose.

We use and disclose client personal information only for the purposes for which the information was collected, except as authorized by law. For example, we may use your contact information to communicate with you about patient support services. We may also use your personal information to provide you with financial assistance as part of a patient support program. If we wish to use or disclose your personal information for any new purpose, we will ask for your consent.
We disclose personal information to unaffiliated third parties only when necessary to provide products or services or as otherwise identified in this Privacy Policy. When we do disclose such information to other parties, we require them to protect Personal Information to the same standard as we use. We protect client personal information in a manner appropriate for the sensitivity of the information. We make every reasonable effort to prevent any loss, misuse, disclosure or modification of personal information, as well as any unauthorized access to personal information. We retain client personal information only as long as is reasonable to fulfil the purposes for which the information was collected or for legal or business purposes. We render client personal information non-identifying, or destroy records containing personal information once the information is no longer needed. We use appropriate security measures when destroying client personal information, including shredding paper records and permanently deleting electronic records.

We rely on our clients to notify us if there is a change to their personal information that may affect their relationship with our organization. If you are aware of an error in our information about you, please let us know and we will correct it on request wherever possible. In some cases we may ask for a written request for correction.

If you want to review, verify or correct your personal information, or find out how it has been used or disclosed, please contact our Privacy Officer using the contact information set out below. When making an access request, we require specific information from you to confirm your identity as well as to search for, and provide you with, the personal information that we hold about you. We may charge you a reasonable fee to access your personal information; but we will advise you of any fee in advance. If you need help in preparing your request, please contact our Privacy Officer. In some circumstances we may not be able to comply with your request to provide access including if it would reveal the personal information of someone else, if it is subject to legal privilege, or where otherwise prohibited by law.
